Transportation Change Form for Pre-K-5th Grade
If your child in Pre-K through 5th grade will be going home in a way that is different from their normal routine, please complete the attached Google Form on the specific day the change will occur.
This form must be filled out each day your child’s dismissal changes—one form cannot be used for multiple days.
